Job Description

Professional Case Management is seeking a compassionate, licensed Physical Therapist to provide high-quality, in-home rehabilitation services to clients with complex medical needs. In this role, you will travel to patients’ homes to perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based therapeutic interventions that maximize mobility, function, and independence. You will work primarily with former energy workers and their families, addressing a wide range of orthopedic, neurological, and chronic conditions. As a key clinical partner, you will collaborate closely with nurses, occupational therapists, physicians, case managers, and family caregivers to coordinate holistic care. Your responsibilities include assessing strength, balance, range of motion, gait, and functional mobility; establishing measurable goals; implementing therapeutic exercise, manual therapy, functional training, and patient education; and modifying plans of care based on progress and clinical findings. You will document all visits accurately and timely using electronic medical records in accordance with regulatory and payer requirements. You will educate clients and families on home exercise programs, safety strategies, proper use of assistive devices, fall prevention, and energy conservation techniques to support long-term health at home. Because services are delivered in the home, you will adapt treatment to varied environments, creatively using available space and equipment while maintaining professional and safety standards.
