Professional Case Management is seeking a compassionate and clinically skilled Physical Therapist to provide in-home therapy services to clients with complex medical needs. As a key member of our Healthcare & Medical Services team, you will deliver one-on-one, evidence-based physical therapy in the home setting, primarily serving former energy workers and their families. You will evaluate functional status, develop and implement individualized treatment plans, and provide ongoing therapeutic interventions aimed at improving mobility, strength, pain management, and overall quality of life. In this role, you will complete comprehensive physical therapy assessments, establish measurable goals in collaboration with clients, families, and the interdisciplinary care team, and regularly reassess and modify treatment plans based on patient progress. You will educate clients and caregivers on exercises, safe mobility, use of assistive devices, fall prevention, energy conservation, and strategies for maximizing independence in daily activities. You will document all care in a timely and accurate manner, ensuring detailed, compliant clinical records and clear communication with nurses, physicians, and case managers. Working within Professional Case Management’s collaborative and compassionate culture, you will participate in care conferences, coordinate services with other healthcare professionals, and advocate for appropriate resources to support each client’s needs. You will provide specialized rehabilitation for individuals with chronic and complex conditions—such as musculoskeletal injuries, neurological conditions, and occupational exposures—using best-practice clinical guidelines and sound clinical judgment. You will manage your schedule to provide consistent, patient-centered visits in the community, traveling to client homes, assessing home safety, and adapting treatment interventions to real-world environments. You will also contribute to quality improvement initiatives, follow all regulatory and safety standards, and model professional, ethical practice in every interaction.
