JOB POSTING Job Summary The Case Manager provides services designed to assist students in optimizing their educational experience and minimizing the impact of illness, injury, and emotional distress on their academic careers. Under the direction of the Associate Director of Integrative Behavioral Health, the Case Manager provides mental health care, case management coordination, and support to the Student Health Services (SHS) and Counseling and Psychological Services (CAPS) clinical providers. The incumbent works with a variety of Division of Student Success (DSAS) departments, provides consultation services for faculty and staff, and coordinates mental health resources with clinical providers, residential life, and the colleges. Appointment Information Budgeted Salary: $8,345.11 per month (10,583.13 for 11 months) Full benefits Full‑time, Fixed; 100%, 40 Hours per Week; Mon‑Fri; Day shift Partial Year Career appointment with 4 week furlough in summer Job Duties Case Management (70%) Performs assessment and consultation to the medical team on the biopsychosocial factors relating to diagnosis and recommended treatment of patients. Works collaboratively with primary care providers, clinic nurses, medical assistants, psychiatrists, mental health providers, and individuals from other clinical units to provide culturally competent care for a diverse student population. Monitors client/patient progress with recommended treatment plans and assures effective coordination of care between providers at CAPS, Student Health Center, hospitals, and other treatment providers. Provides case management support to primary care providers at the Student Health Center regarding mental health and physical health needs. Serves as liaison between the student, hospital, treatment programs, campus, and community resources; develops and aids in the use of community resources; maintains current knowledge of University Health Services, campus, and community resources. Professionally responsible for the soundness and validity of recommendations, services, and referrals, but is expected to seek advice from supervisor on unusual or uniquely complex client responses or on problems outside their area of expertise. Assists with monitoring and tracking students as they move through the mental health system, which includes Counseling and Psychiatry, hospitalization, and referral to off‑campus outpatient treatment. Provides triage crisis assessment and crisis intervention. Consultation & Outreach (15%) Provides consultation to other departments, staff, administrators, and faculty to enhance the social, personal, and academic functioning of students with severe psychological difficulties. Functions as a resource and may provide educational and consultative services to groups and organizations. May participate in on‑campus/community groups and consortia to improve efforts for integrated behavioral health care. Counseling & Clinical (15%) Provides brief counseling and psychotherapy with undergraduate and graduate students (individuals, couples, and groups) with a broad range of co‑occurring medical and mental health needs. Performs behavioral interventions, motivational interviewing, brief psychotherapy, and case management on a full range of cases, including high‑risk concerns. Provides coordination and consultation for After‑Hours Crisis Services as scheduled. Required Qualifications Advanced degree in a related area and/or equivalent experience/training. Valid California license in good standing in Social Work, Marriage and Family Therapy, or as a Licensed Professional Clinical Counselor. Expertise in clinical case management in a medical or integrated behavioral health setting. Strong knowledge and experience providing psychological diagnoses, brief interventions, and case management services to university students, and the ability to rapidly provide these services while working in a fast‑paced environment. Demonstrated multicultural competence in mental health service provision. Broad skills and competencies associated with providing multicultural counseling services and experience with communities underserved in mental health care. Experience providing service to a young adult population. Solid familiarity with a wide range of psychological interventions, including harm reduction, assessment, crisis intervention, crisis management, and brief psychotherapy. Demonstrated ability to work as part of an interdisciplinary team of mental health and health providers. Demonstrated skills and experience in developing liaison relationships and consulting with diverse faculty, staff, public and private mental health providers, and students regarding student mental health issues. Highly effective written and verbal communication skills. Ability to work in a highly collaborative manner. Flexibility and willingness to accommodate client needs and colleague requests for support. Thorough knowledge of applicable laws and standards of professional conduct. Experience in the use of electronic/medical records systems. Must be computer literate. Preferred Qualifications Experience as a case manager in a behavioral health care setting (e.g., Behavioral Health Unit, Day Treatment Program, Outpatient Mental Health Center). Experience providing behavioral health interventions in a medical setting. Demonstrated ability to work with required computerized clinical record keeping systems. Counseling experience in a college/university setting. Demonstrated knowledge of a university environment and the developmental issues typically encountered by college students. Special Conditions of Employment Valid California license in good standing in Social Work, Marriage and Family Therapy, or as a Licensed Professional Clinical Counselor. Must successfully complete a pre‑hire criminal history background check and a pre‑hire credentialing check. Must pass the employment misconduct disclosure process. Must possess CPR/First Aid/AED certification or have the ability to obtain it within 60 calendar days of hire. Must take a TB test within 60 days of hire. Must complete training within established time frames, including UC compliance training and Health Insurance Portability and Accessibility (HIPAA) training within 30 days of hire. Work primarily on campus, with remote work possible based on operational needs; if a hybrid schedule is confirmed, must be able to work successfully from a home/remote office and fulfill the UCSC telecommuting agreement. Must comply with current Student Health Center employee health safety requirements for health care workers as defined by state and federal regulatory agencies. Must maintain a professional appearance and conduct suitable for the workplace. Must comply with the University of California vaccination policy.
